Welcome to Our 16+ Outreach Service. We provide tailored support for young people aged 16 and over, helping you take the next step towards independent living with confidence. Governed by the Office for Standards in Education, Children’s Services and Skills (OFSTED), we deliver the highest standards of care while guiding you through every stage — from moving into your own home to building the skills you need for a successful future.
We work alongside you to plan and prepare for life on your own. Once you’re ready to move into your own accommodation, our outreach team can reassess your support needs and continue working with you while you settle into your new home.
We coordinate with a range of agencies to make sure you get the right help — from finding the right place to live, to managing finances, to maintaining your health and wellbeing.
Every young person is different, which is why we develop tailored care and pathway plans with you. This collaborative process involves you, your key workers, and your social worker, ensuring your plan is practical, achievable, and right for your future.
You’ll take part in Life Skills sessions with a key worker to build the skills you need for independence — budgeting, cooking, time management, and understanding your rights and responsibilities.
We’ll be there during the early stages of your move, accompanying you to view new placements and offering guidance to make the change as smooth and stress-free as possible.

Our aim is to make sure you are prepared, confident, and equipped with the skills you need to thrive when you leave care and step into independent living.
We accept referrals for young people aged 16 and above who would benefit from outreach support to sustain independence in the community.
Our referral process ensures that every individual receives the right level of intervention and continuity of care.
Referral Pathway:
Referrals are accepted from local authorities, leaving care teams, or partner agencies.
Each referral is reviewed to ensure our model of support meets the young person’s needs safely.
A personalised engagement plan is agreed, setting visit frequency, communication methods, and key goals.
A Key Worker is assigned to coordinate progress reviews and liaise with professionals involved.
Stability and Review:
We maintain consistent contact and adapt support as the young person’s independence grows.
If additional needs arise, we work collaboratively with the placing authority to review and adjust support intensity.
